The Birmingham and Solihull Community Care Collaborative (CCC) is a partnership between primary care, community health services, community mental health services, social care and the community and voluntary sector.
Its aim is to tackle local challenges, improve patient outcomes and enhance resilience by providing better joined up health and care services in the community for people living in Birmingham and Solihull.
This will make it easier for people to access the care they need, when and where they need it and by the right person. It will also enable people to stay well for longer and more independently, in their own homes.
The initial three-year focus of the CCC will be across five workstreams:
- Locality Hubs, incorporating Integrated Neighbourhood Teams
- Intermediate Care services such as urgent community response, virtual wards and remote monitoring
- Long Term Conditions
- Primary Care Development
- Children and Young People’s community services.
Birmingham Community Healthcare Trust (BCHC) has been appointed by the Birmingham and Solihull Integrated Care System to lead and deliver the CCC programme of work.
