What is a Personalised Care Plan and how do I receive this?

Personalised care means that women, birthing people and their families have choice and control over the way their care is planned and received based on 'what matters' to them. It takes into account individual needs and preferences to tailor their care. The healthcare professional will consider and discuss their home, family, health and wellbeing, and birthing preferences, so that the care reflects their needs and wishes.

Shared decision making in the NHS is a joint process and is the principle that underpins the Personal Care Plan (PCP). It means that anyone receiving care is fully supported and informed to understand the options, decisions, and care that they will receive.

The Badger Notes App access is provided by your midwife at your first booking appointment and is the tool through which the PCP can be accessed by you, and can be shared with your GP, so that they are fully informed of the care plan and able to interact with it.
